Entry Rules: Winners chosen at random. Only one entry will be accepted per household per month. Prizes will be shipped/mailed to the address entered on this form, so please enter it carefully. Winners will be notified by e-mail and must respond within 7 days of the actual drawing date in order to claim prizes. After 7 days a new winner will be chosen at random. The process will be repeated every 7 days until the prize is claimed.
